Record Book Forms

The 4-H Record Book Personal Information Page, Report Form, and Checklist is now all in one publication. This is the form used in completing a competitive record book. It is available in PDF form from the  link below and is also available for download in Word Perfect and Microsoft Word formats:

Record Book To Do List

  • Recordbook forms must be signed by a Club Manager or a Project Leader before they are turned in. UNSIGNED forms in your recordbook will not be accepted. Project forms require a leader’s signature.

• 4-H Member Recognition Application form (pink) must be clipped to the outside front cover of your Recordbook or attached to the front of the project form. The 4-H Member Recognition Application form must be signed by your Club Manager or it will NOT be accepted. A member receives a year pin by submitting a project form only. A member receives a year pin and a project medal when the completed Recordbook is submitted. If you need a copy of the Member Recognition Application contact your Recordbook Leader.

• Complete the top portion of the Recordbook Check Sheet. Clip it to the inside of the front cover of your Recordbook.

The Texas 4-H WEB Site has added a new section called "For the Record, Texas 4-H Recordkeeping System". Click on the link to visit this new section. This new section includes information about the forms, the categories, and answers to the Most Frequently Asked Questions. You can also get the same forms by following the links below.

By downloading these files you can obtain the newest information about Record Books. We recommend clicking on each link and then saving the file to your own hard drive.
